Summary:
This series includes master microfilm copies of original terminated case files of clients served by the Craig Developmental Disabilities Services Office (CDDSO). The records filmed chronicle all services provided to clients, including administrative, legal, and programmatic. As such they provide clinical and administrative information documenting the state's institutional services to a substantial number of its developmentally disabled population.

Scope and Content Note

16437-94: This microfilm is master negative copies of original terminated case files of clients served by the Craig Developmental Disabilities Services Office (CDDSO). The records filmed chronicle all services provided to clients, including administrative, legal, and programmatic. As such they provide clinical and administrative information documenting the state's institutional services to a substantial number of its developmentally disabled population.

The facility is the successor to the Craig Colony for Epileptics and the Craig Developmental Center. Files dating from 1967 reflect a change in client population; although it continued to serve a large number of epileptics, new admittees were drawn from a geographic service area, not from a particular class of mentally-impeded people.

The first subseries (278 rolls) includes case files dating from 1950 through 1969. Inclusive file numbers are 8049 through 15970. The date that appears on the microfilm boxes in this subseries (ranging from January through June 1992) apparently refers to when the records were filmed. There are no boxes with original box numbers 101-124, but file numbers are not affected.

The second subseries (102 rolls) includes case files from 1960 through 1980. The boxes show no date information. Inclusive file numbers are 1462 through 12872.

16437-97: This accretion consists of original microfilm of medical discharge records of clients of the Craig Developmental Disabilities Services Office. Each reel has three labels. The top label ("DDSO Medical Discharge") shows date, department ("Medical") and reel box number. A front label shows the series title, span dates, box number, and inclusive file numbers. An "Indexing" label shows "case" and corresponding "seq." numbers.

Reels in box 1 are numbered from 1 to 25-26. Reels in box 2 are numbered from 101 to 124-125. A transmittal letter states that the reels cover the period 1950-1969.

Custodial History

16437-97: The original microfilm was transferred to the State Archives in March 1992 without use copies. The accession resulted from a project by Archives staff in 1997 to accession or refile estrayed or unidentified records. Production of use copies is contingent on acquiring adequate resources.

Access Restrictions

Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office for People with Developmental Disabilities.
