Scope and Content Note

These are records of incarcerated individuals who died in prison. Case files were kept for all incarcerated individuals, and records of deceased individuals were pulled from the active files and filed together.

They include records directly related to the incarcerated individual's death, such as Department of Health burial permits; telegrams to relatives notifying them of the individual's death; discharge-by-death notices; orders to note death in the records; and copies of the death certificate. The files also contain any records that had been kept on the incarcerated individual, including, occasionally, records from the other institutions in which he had been housed. The records may include fingerprint and Bertillon cards; psychiatric reports; psychologists' reports; medical histories; requests for correspondence and visitation by relatives; lists of correspondents and visitors allowed; physical examination reports; disciplinary action notices; records of clothing issued; Sunday letter correspondent records; correspondent report cards; accounts of funds on deposit; incarcerated individual's institution history summaries; records of visitors; requests to spend reserve compensation money; reports of injury; applications for executive clemency; Discretionary Reduction of Sentence Not Earned forms; warrants for execution; executive orders; notices of appeal; copies of sentence to prison and orders of imprisonment; personal property records; mug shots and negatives; conduct and punishment records; copies of receiving blotter page; classification clinic reports; and any other records that had been made concerning the incarcerated individual.
