Summary:
This series consists of printed forms compiled by the State Department of Health's Division of Laboratories and Research. The reports confirm the presence of typhoid fever and provide the following data: laboratory number; date; source from which blood was taken; patient's name and address; date the sample was taken; doctor's name; institution address; results of tests; and health officer's name and address. The back of the form provides an interpretation of the results. Records are restricted pursuant to state and federal law.

Administrative History

The records were generated in response to Section 25 of Chapter 45 of the Consolidated Laws of 1909 (L. 1909, Ch. 49). This law required all physicians to immediately notify the State Department of Health of every case of "infectious and contagious or communicable" disease. The tests were then performed by the department's Division of Laboratories and Research.

This series consists of 7" x 5 1/2" printed forms compiled by the Health Department's Division of Laboratories and Research. The reports confirm the presence of typhoid fever and provide the following data: the laboratory number; date; source from which blood was taken; patient's name and address; date the sample was taken; doctor's name; institution address; results of tests; and health officer's name and address. The back of the form provides an interpretation of the results.
