Arrangement
Organized into five subseries: 1. Pharmacists; 2. Pharmacist Exchange; 3. Druggist; 4. Duplicate licenses; 5. Substitute licenses.
By type of license, then either numerically by license number or reverse chronological order by date license sent. See narrative for details.
Administrative History
The law provided for examination and granting of two grades of licenses: "licensed pharmacist," who could prepare and dispense drugs and poisons anywhere in the state; and "licensed druggist," who could prepare and dispense drugs and poisons only in communities having 500 or fewer inhabitants. Persons licensed by predecessor boards as assistant pharmacists or pharmacists could exchange their old licenses for "licensed druggist" or "licensed pharmacist" licenses, respectively.
Scope and Content Note
These volumes contain a record of pharmacist and druggist licenses issued by the State Board of Pharmacy. The first volume is divided into sections by type of license, and the remaining volumes are name indexes to parts of volume 1.
Pharmacists, 1901-1910: Licenses granted to pharmacists by all branches of the board (Eastern, Middle, Western) upon examination or in exchange for licenses granted by predecessor boards. The following information is given for each license: name and address; branch granting license; date of examination or exchange; and license number. In the case of licenses exchanged, the granting board, date, number of former license, and basis on which it was granted (e.g. examination, experience) are given. The first three or four pages are missing (license numbers 1-48).
Pharmacist Exchange, 1907-1910: Licenses granted by all branches in exchange for licenses granted by predecessor boards, and designated "Series B." Entries contain the same information as described licenses in the previous section.
Druggists, 1901-1910: Licenses granted to druggists by all branches of the board upon examination or in exchange for licenses granted by predecessor boards. Entries contain the same information as those for pharmacist licenses in the first section.
Duplicate Licenses, 1903-1910: Duplicate pharmacist and druggist licenses issued to replace licenses of the statewide board lost or destroyed. Duplicates have the same license number as the original. Each entry gives the following information: name and address of license; license number; cause of loss or destruction (or other reason for duplication, such as name change); and date duplicate sent. These entries are arranged chronologically by the date the duplicate was sent. The indexes to the pharmacist and druggist licenses are annotated to indicate the issuance of duplicate licenses.
Substitute Licenses, 1901-1910: Pharmacist and druggist licenses granted by all branches to replace licenses originally granted by predecessor boards and subsequently lost or destroyed. Substitute licenses were issued with new numbers. Entries contain the same information as those for duplicate licenses, except the date sent is not given.
