Summary:
This series consists of notes from staff meetings at Willard State Hospital. The notes document staff presentations and discussions of cases of individual patients. Information includes date; staff in attendance and persons giving reports and summaries. Patients discussed were sometimes at the meetings. Their statements and staff's observations of them are included. Reports include case abstracts; patients' social, physical, and mental histories and diagnoses; treatment histories; and present symptoms. Diagnoses and parole decisions are sometimes noted. Records are restricted.

Scope and Content Note

These volumes contain handwritten notes from staff meetings at Willard State Hospital. The notes document staff presentations and discussions of cases of individual patients. Information given includes date; times of beginning and ending of meetings; staff in attendance; and names of persons giving reports and summaries. Patients whose cases were being discussed were sometimes present at the meetings, and their statements and the staff's observations of them are included. Reports from staff include abstracts of cases; patients' social, physical, and mental histories and diagnoses; treatment histories; and present symptoms. Staff determinations concerning diagnoses and parole decisions are sometimes noted.

B1463-02: This accretion (1941-1960) consists of typed-written notes and contains information similar to the earlier accretion.

Custodial History

These records were transferred to the State Archives from Willard Psychiatric Center at the time of its closure in April, 1995.

Access Restrictions

Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
