For each patient account, articles issued are listed by date. A price for each article is given and a number, which is presumably the patient's individual account number, is assigned.

Custodial History

These records were transferred to the State Archives on September 16, 1996 from Mohawk Valley Psychiatric Center under RDA 19266.

Access Restrictions

Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
