For each county the form shows who conducted/submitted the record, with the date; county of the facility; examination number (from as few as none to as many as 10); and answers to a series of 26 questions on child identity, personal/family history, siblings, economic circumstances of parents, moral character, mental health, causes of dependency, and the likelihood of future independence. Often a section for remarks provides additional information.
