Summary:
This series consists of logbooks recording information related to x-rays taken of patients and employees of Utica State Hospital (later Utica Psychiatric Center). Each entry typically records date, number, name, and area of the body x-rayed. Some volumes contain only lists of patients to be x-rayed and the dates of scheduled appointments. The volumes may have been created to track the status of patients who had received x-ray treatment, possibly for followup.

Scope and Content Note

This series consists of logbooks recording information related to x-rays taken of patients and employees of Utica State Hospital (later Utica Psychiatric Center). Each entry typically records date, number, name and area of the body x-rayed. Some volumes contain only lists of patients to be x-rayed and the dates of scheduled appointments. There are no actual x-rays in the series.

A second set of volumes marked "census" list patients in roughly alphabetical order with a date (post-dating the appearance of the name in the logbook) and status (expired, CC, FC, or discharged). Exact purpose of these volumes is uncertain, but it may be inferred that they were created to track the status of patients who had received x-ray treatment, possibly for followup.

The remaining volumes in the series consist of logbooks for patients receiving x-rays at other facilities; for x-rays of employees, potential employees, and students; for x-rays of patients from other hospitals; and of appointments for x-ray rechecking in Albany.

These records were transferred to the State Archives on May 28, 1997 from Mohawk Valley Psychiatric Center under RDA 19266.

Access Restrictions

Patient and employee medical information is restricted in accordance with Section 33.13 of the Mental Hygiene Law (relating to confidentiality of clinical records), by Section 18 of the NY Public Health Law, and the US Health Insurance Portability and Accountability Act. Access permitted under certain conditions upon application to and approval by the Office of Mental Health.
