Alphabetical by name of facility.
Scope and Content Note
This series consists of copies of reports made by the directors of several mental health institutions to their respective boards of visitors and forwarded to the Governor's office. Reports contain a variety of narrative and statistical information including incident reports, institutional censuses, goals and objectives, and personnel status. Transmittal letters from the directors to the governor are usually included. Names of patients appear throughout the series.
Custodial History
These reports were apparently withdrawn from series 13682, Central Subject and Correspondence Files, and maintained as a separate series by Executive Chamber staff.
Access Restrictions
Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
