Scope and Content Note
This series consists of accident, injury, and escape reports prepared by the ward physician and other staff and submitted to the superintendent. The reports vary slightly over time but usually include date of report, name of patient, patient number, date of admission ward, mental diagnosis, age, and date and time of incident. The accident reports typically include patient's general mental and physical condition, nature and cause of accident, physician's findings, treatment, witnesses, and persons notified. The escape reports include manner of escape and law enforcement agencies notified.
