This series documents patient admissions, discharges, and deaths. The records may have been used for reporting purposes or for the compilation of statistics. Information includes patient admission, re-admission, discharge, and death cards and weekly lists of patients discharged and admitted. Weekly lists vary in content from detailed to mere lists of patient names, date, sex, patient numbers, and manner of leaving for discharges or legally liable relative for admissions. Scope and Content Note

This series consists of scattered documentation of patient admissions and discharges, and deaths. Series includes patient admission, re-admission, discharge, and death cards (ca.1941-1945); weekly lists of patients discharged (April 1960-March 1964); and weekly lists of patients admitted (1943, 1964-1965). Cards include detailed patient information. Weekly lists vary in content from detailed to from mere lists of patient names, date, sex, patient numbers, and manner of leaving for discharges or legally liable relative for admissions. Information contained in these records was possibly used for reporting purposes or for compiling statistics.
