Summary:
This series consists of approximately 490 weekly census reports of veterans of the United States armed forces who were patients at Utica State Hospital. Each weekly report form consists of a single, two-part, printed sheet. Table "A" shows categories for general, tuberculosis, and neuro-psych patients. These are carbon copies of the original forms which were apparently prepared for and submitted to the United States Veterans' Bureau for purposes of establishing benefit eligibility for hospitalized veterans.

Scope and Content Note

This series consists of approximately 490 weekly census reports of veterans of the United States armed forces who were patients at Utica State Hospital. Each weekly report form consists of a single, two-part, printed sheet. Table "A" shows categories for general, tuberculosis, and neuro-psych patients. In these reports, all patients are listed as neuro-psych. Statistics are given for number of patients remaining at end of previous week; admitted during the week reported; discharged during the week; died during the week; and remaining at the end of the week.

Table "B" provides space for name of beneficiary (patient); claim number; home address; rank and organization; diagnosis; date of admission; date of discharge or death; and reason for discharge. These are carbon copies of the original forms which were apparently prepared for and submitted to the United States Veterans' Bureau for purposes of establishing benefit eligibility for hospitalized veterans.

These records were transferred to the State Archives on May 28, 1997 from Mohawk Valley Psychiatric Center under RDA 19266.

Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
