Scope and Content Note
This series consists of roughly 30 incident reports, documenting official investigations into complaints of charges of misconduct or neglect of duty by staff members. The complaints originate from patients, former patients, and staff members. Record types include correspondence from former patients and notifications of hearings, personnel suspensions, and terminations.
Types of incidents include escapes, accidents, injuries to staff members and patients, intoxication on duty, gambling, inappropriate use of force in restraining patients, and other inappropriate conduct.
Access Restrictions
Records identifying patients are restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
