Scope and Content Note
This series consists of carbon copies of printed forms containing raw statistical data on individual female patients. Space is provided for name, legal status of commitment, identification number, date of admission, psychiatric classification, significant disorders, parents, education level, household composition, drug and alcohol use, and other information. Purpose and results of survey are unknown.
Access Restrictions
Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
