Scope and Content Note
Series consists of 16mm silver halide negative microfilm of original patient case files (1998-2001) maintained by thirty-three mental health facilities. A typical file contains admission/commitment papers, various legal documents, psychiatric evaluations, patient case summaries, correspondence, medical notes/observations, and discharge related documents. Modern case files, depending on the length of an individual's association with the Office of Mental Health, may contain between fifty to several hundred documents.
