The following information is contained: salary; time period covered; annual salary; retirement registration number; relevant law and chapter; title of position; rate of contribution; amount of contribution; amount paid employee; and check number. Also for the Department of the State Engineer and Surveyor are printed forms of schedule of accounts of the Canal Fund, listing: appropriation; chapter and laws; claimant; voucher number and amount; requisition number and amount; and check number.
