Summary:
This series consists of affidavits submitted by teachers in application for replacement of state teaching certificates that
were lost or destroyed. Most affidavits are on standardized forms providing date and location of completion of work under
the uniform system of examination, date and location in which teaching certificate was issued, the grade of the certificate,
and date and manner in which the original certificate was lost or destroyed.
