Summary:
This series consists of blank medical forms used by custodial institutions for mentally deficient inmates. The majority of
the forms are from New York institutions; most were collected from the Craig Colony for Epileptics. Forms include admission
applications; personal and family histories; medical and dental examination charts; testing and other school charts; diagnosis
forms; institutional reporting forms; death records; and agreements between guardians and institutions relating to custody
of inmates.
