Scope and Content Note
This series identifies individuals applying for and taking pharmacist licensing examinations.
The following information is given about each applicant: application number (assigned sequentially according to type of certificate applied for); name; form of blank (identified by a number); dates application received, examined, and approved; dates examination given and approved; name of examiner and city where examination given; kind of examination; percentage; certificate number; form number; date certificate issued; and remarks (usually filled in with town, city, or village).

Volume 3 indexes volumes 1 and 2, arranged alphabetically by the first two letters of the applicant's last name. The second column under heading "Pages" indicates the page number in volumes 1 or 2. (The numbers in the first column are unidentified.) Applicant's name appears more than once if different kinds of certificates were applied for by the same person. Index also gives applicant's address.
Container list gives dates of each volume.
