The series consists of use copies of microfiche only. Original certificates indexed by the fiche, as well as master copies of the fiche indexes, remain in the custody of the New York State Department of Health. Two sets of copies are made available to the State Archives by the Department of Health for research use.
The New York State Archives is part of the Office of Cultural Education, an office of the New York State Education Department.
