Scope and Content Note

This series consists of indexes to patient case numbers and indexes to patient casebooks created by the staff of Middletown State Homeopathic Hospital. For some patients an identification number accompanies the case number, the reference of this number is unknown. Dates for the indexes overlap as information was carried over from earlier to later volumes.

Subseries 1, Indexes to case numbers. Indexes are arranged alphabetically by surname, except for Volume 5, which is arranged sequentially by case number and then name. Information given in Volume 1 (1874-1875) includes name and case number for male and female patients. This volume is alphabetical by last name. Volume 2 (1874-1895) contains case numbers (in red ink) and casebook page numbers (in black ink) for male and female patients. Volume 3 (ca. 1880-1930) lists, for male patients only, information in the following order: ward number, name, case number, whether transferred, and if admitted as an emergency case. Volume 4 (1917-1929) lists, for male patients only, information in the following order: case book number, name, county, and other identification numbers of an undetermined purpose. Volume 5 (ca. 1876- ca.1930) lists, for female patients only, case number and name.

Subseries 2, Indexes to patient case books. The first and third indexes (1874-ca.1893 and 1887-ca. 1900) are for male patients; the second and fourth indexes (1874 - ca. 1900 and 1887 - ca. 1900) are for female patients. All volumes in this subseries are alphabetical by patient surname. Information includes name, casebook volume letter (casebooks, Series 14231-01, are often missing their spines where these letters appear), casebook page, then date of admission and discharge.
