Resident health questionnaires and medical records are restricted in accordance with Public Health Law § 206 (l)(j), Personal Privacy Protection Law § 96 2 (b) and the Health Information Portability and Accountability Act (HIPAA). State Archives staff will review remaining documents and disclose information pursuant to the Freedom of Information Law.
