Scope and Content Note
Register contents vary overtime but typically include date; name of attendant reporting; names of employees present; numbers of students present; number of vacancies; time and name of supervisor visiting; other visitors, shoes, and wraps checked; numbers of students moved to engage in various activities; documentation of accidents, illnesses, etc. and their treatment; other significant incidents; and time rounds made by attendant and number of students counted. A separate report is present for each shift (day, evening, night) in each twenty-four hour period.
