The series consists of survey files divided into official documents (white tab) and background documents (yellow tab). White tab files contain survey report, the statement of deficiencies, plan of correction, and related correspondence and reports. Yellow tab files contain background, surveillance, or informational material used to produce the final survey report.
The New York State Archives is part of the Office of Cultural Education, an office of the New York State Education Department.
