Scope and Content Note
The series consists of blank survey forms used by the Commission to examine statewide needs and current programs for the care and training of mentally deficient persons. The majority of the forms relate to surveys sent by the Commission to schools and custodial institutions in the state asking for information on mentally deficient students and inmates and on the care and training provided.
The New York State Archives is part of the Office of Cultural Education, an office of the New York State Education Department.
