This series consists of blank survey forms used by the Commission to Investigate Provision for the Mentally Deficient to examine
statewide needs and current programs for the care and training of mentally deficient persons. Most forms relate to surveys
sent to schools and custodial institutions in the state requesting information on mentally deficient students and inmates
and on care and training provided them.
