These reports describe "incidents" where dangerously unusual behavior was displayed by a patient or staff member. Reports
are present from both the Nursing and Quality Assurance Offices. The Quality Assurance Office reviewed reports to ensure that
staff were properly following policies and procedures; some reports (particularly those involving major injury or death) were
additionally reviewed by the Board of Visitors for final response.
